Could you take our word version of our enrollment form and turn it into a clean, professional looking form? In addition, if you have experience in this, and have any recommendations as to the wording, I'm open to changes ... but mostly we are just looking for clean spacing, and fitting this information comfortably and professionally on one page.
What's your vision?
Accepted File Formats: PDF (300 DPI) as well as original Illustrator, inDesign, etc. Please include fonts. Please also place our logo on the page. Both the logo and enrollment form copy can be found here -- http://www.legaldirectives.com/temp/ It would also be great if we could include 3 emergency contacts rather than 2, but this is not strictly necessary. Thanks!
